Fertility at ANDORRA

It is estimated that infertility affects 1 in 7 couples worldwide, including Malaysia. The fertility rate (birth per woman) has been declined over the years. In 1970, the birth rate per woman was noted to be 4.9, however in 2018, for the first time it has reach a negative balance of 1.9 per woman. It is estimated that the birth rate will further decline to 1.7 per woman in year 2050. The increase in the prevalence of fertility problems will lead to greater proportion of people now seeking help for such problems.

In about 40% of cases disorders are found in both the man and the woman. Uterine or endometrial factors, gamete or embryo defects, and pelvic conditions such as endometriosis may also play a role.

Given the range of causes of fertility problems, the provision of appropriate investigations is critical. These investigations include semen analysis; assessment of ovulation, tubal damage and uterine abnormalities; and screening for infections such as Chlamydia trachomatis and susceptibility to rubella.

Once a diagnosis has been established, treatment falls into 3 main types:

  • Medical treatment to restore fertility (for example, the use of drugs for ovulation induction).

  • Surgical treatment to restore fertility (for example, laparoscopy for ablation of endometriosis).

  • Assisted Reproduction Techniques (ART): any treatment that deals with means of conception other than vaginal intercourse such as IUI or IVF. It frequently involves the handling of gametes or embryos.

The main causes of infertility are (per cent figures indicate approximate prevalence):

  • Unexplained infertility (no identified male or female cause) (25%)

  • Ovulatory disorders (25%)

  • Tubal damage (20%)

  • Factors in the male causing infertility (30%)

  • Uterine or peritoneal disorders (10%).

Intraurine Insemination (IUI) Package

Intrauterine Insemination (IUI) is a fertility procedure that involves inserting sperm within the uterus of a woman to promote fertilisation. The aim of the IUI is to increase the amount of sperm that enter the fallopian tubes and, consequently, to increase the chances of fertilisation. IUI gives the sperm an advantage by giving it a head start, but it also allows thesperm to penetrate and fertilise the egg on its own. This is a less invasive and less costly choice  compared to In Vitro Fertilization (IVF).
